If you require a copy of your medical records or any forms please review the list below for instructions on how to obtain the information or contact our Correspondence Department at (302) 529-8783 press number 1 to enter the extension 1210 or choose option 6.
Patients- Patients requiring a copy of your medical records must come into the office to sign a Release of Information form. Below are the fees.
- $2.00 per page for pages 1-10
- $1.00 per page for pages 11-20
- $0.90 per page for pages 21-60
- $0.50 per page for pages 61 and above
Doctors/Continuation of Care- If a doctor or medical facility is requesting the information, a signed release must be faxed or emailed to (302) 529-1586 or correspondence@delawarebackpain.com. Please ensure that an address and/or fax number is on the request.
Lawyers/Insurance Companies/Third Party Companies-Any attorney or insurance company requiring medical records and/or any other requirements must fax or email with a signed authorization to (302) 529-1586 and/or correspondence@delawarebackpain.com. If it is a billing statement please fax the request to (302) 832-5854 or contact the billing department at (302) 832-3369.
In the event of a life-threatening medical emergency, call emergency medical services at 911 immediately. DO NOT WAIT! Inform the emergency room staff that you are a patient of Delaware Back Pain, and they will contact us.
If you have an urgent medical need, and need to talk to our doctors after our regular hours, please call the office location you usually visit. A doctor is on-call 24 hours a day. Your call will be returned promptly.
If you are having a medical emergency, please dial 911.
At Delaware Back Pain & Sports Rehabilitation Centers, we accept Workers’ Compensation, Medicare, and most major insurance plans. Please contact your insurance company, or call the office to find out if we accept your insurance plan.
If your insurance plan requires you to pay a co-payment, co-insurance, and/or a deductible, you will need to pay at the time of your visit. For your convenience we accept cash, checks, Visa, MasterCard, Discover, and American Express.
If you have a question related to your bill or insurance, please ask to speak with a billing specialist when you call.
If your insurance is an HMO/managed care plan and you are required to obtain a referral from your primary care physician, the referral must be obtained before you can be seen for an office visit. Please coordinate this with your primary care physician in advance. Appointments will be rescheduled if required referrals are not received in the office by the time of your visit.

Prescriptions and refills for established patients of Delaware Back Pain & Sports Rehabilitation are routinely issued during our regular office hours (see above). You can make your request by calling the office.
No refills are issued during the evenings, weekends, or holidays, as the doctors do not have access to your medical records during those times. It is helpful to anticipate your refill needs to avoid an interruption in your medication therapy.
